Enrollment Requirements

Enrollment will not be complete until all items below are received.

• Enrollment Form

• Tuition Form

• Registration fee and minimum of 1 quarterly tuition payment

• Copy of Birth Certificate

• Copy of Valid Parent/Guardian Photo ID

• Copy of Proof of Current Immunizations or completed waiver  

• Copy of most recent Report Card or Unofficial Transcript
